about my medical records?

You can have your medical records sent to
us before your first visit. All that
your current physician should require is
a signed request/consent from you ("Please
transfer my medical records to...") and
our name and address. To request a transfer
of your records can be completed at either
our office or through your current physician's
office.

and medical assistants schedule patients
approximately two weeks in advance.
If there is a special need, make sure to
